Claim Procedure

· Health Cashless

  • Insured to contact Alarm Centre of our TPA, Paramount and its Assistance Co -operation Partners through Paramount Alarm Centre  91-022-67515515/91-022 -66620863/91-022-40004219 or From USA 001 866 9785205
  • Show the policy to the physician if required
  • Paramount will call for basic medical records and scrutinizes the same
  • Additional documents, if any required, will be called for and decision on cashless approval or rejection is   conveyed to the hospital

Health Reimbursement

  • Contact Alarm Centre of Paramount and its Assistance Cooperation Partners through Paramount Alarm Centre 91-022-67515515/91-022-66620863/91-022-40004219 or From USA 001 866 9785205
  • Submit the duly filled in claim form along with original discharge summary bills/prescriptions / reports / X ray receipts and any other relevant documents to Paramount.
  • In case of death, official death certificate has to be submitted
  • For extra expenses for transportation of insured person to India, a statement issued by the attending physician indicating cause of illness and the necessity for transportation is required.
  • Documents scrutinized, additional documents if any required would be called for and decision on the claim is conveyed.
  • Cheque will be despatched to you if the claim is admissible, otherwise a denial letter will be sent.

Loss of Baggage/ Delay in Checked-in baggage

  • Submit the duly filled in claim form along with Boarding Pass / baggage coupon, Property irregularity report, letter from airlines regarding loss and any compensation made, declaration about the items lost with cost, if the cost of more than 100 USD, proof of ownership, bills/vouchers etc any other relevant document.
  • Royal Sundaram will depute a service provider (if required), to verify the records/
    circumstances of the claim.
  • Documents scrutinized, additional documents if any required would be called for.
  • Cheque will be despatched to you if the claim is admissible, otherwise a denial letter will be sent.

· Personal Accident:

    •  Submit the duly filled in claim form along with Death certificate, FIR, Post Mortem.
    • Report, Discharge summary, news paper cuttings if any, any other relevant documents.
    • Royal Sundaram / Paramount may depute a service provider to verify the documents/circumstances of death.
    • Documents scrutinized, additional documents if any required would be called for.
    • Cheque will be despatched to you if the claim is admissible, otherwise a denial letter will be sent.
